Heeft u klachten bij het plassen? Dan kunt u uw urine laten testen. Vul daarbij onderstaand formulier in. 

Vang de urine op in een schoon potje (deze kunt aan de balie bij ons gratis meenemen). Vangt het liefst ochtendplas op. Breng het potje urine binnen 2 uur naar de praktijk, of zet het in de koelkast en breng het binnen 24 uur. Vul dit formulier in en lever de urine in vóór 11:00 uur bij de assistente. 

Let op! Wanneer u de urine overhandigd aan de assistente, vermeld altijd wie u bent, of u het formulier online heeft ingevuld of ter plekke wilt invullen aan de balie.  Zo weten wij zeker dat wij het juiste onderzoek voor de juiste persoon doen. Bedankt voor uw medewerking.

URINE TEST

Do you have symptoms when urinating? If so, you can have your urine tested. Fill out the form below.

Collect the urine in a clean container (you can get one for free at our reception desk). Preferably collect the morning urine. Bring the container to the practice within 2 hours, or store it in the refrigerator and bring it within 24 hours. Fill out this form and submit the urine to the assistant before 11:00 AM. 

Please note! When handing your urine container to the assistant in person, always inform who you are and whether you have completed the form online or wish to fill it out at the desk. This ensures we perform the correct test for the right person. Thank you for your cooperation.
